An Oregon Medical Power of Attorney lets you assign an agent to make medical decisions if you become incapacitated and can’t make those decisions yourself.
Definition: An “Advance Directive” includes a form appointing a health care representative and instructions to the representative, along with any supplementary document or writing attached by the principal (ORS 127.505(2)).
Signing Requirements: Two (2) Witnesses or a Notary Public (ORS §127.515).
Revocation: Make a new one with different conditions, notify your doctor, or create a written revocation form, and if your spouse is your agent, getting a divorce or annulling your marriage will suspend it (ORS §127.515).
